Care Team Referral
Use the button below to complete a form to refer students who may benefit from additional support in the areas of academics, attendance, behavior, or social-emotional well-being.
Any teacher, staff member, or parent/guardian may complete this form.
💡 Please note: This is not a disciplinary referral. It is a request for support.
Once submitted, the Smith Elementary Care Team will review the referral and determine next steps. Depending on the student’s needs, support may be provided by the school counselor, Communities In Schools (CIS), or our Integral Care therapist. A team member will follow up as needed. Thank you for helping us review the whole child.
